As team leader at privately owned ambulance service Medical Safety Team (MST) Thailand, which caters largely to mentally ill individuals, he learns that members of the patient's family restrained the hallucinating man, who is profusely bleeding from a self-inflicted wound but refuses to go to the hospital, before calling MST as their last resort.
